I think I remember reading that pic was taken while Rock in Rio in 91 or it may have been a few days before. Besides, Axl married Erin Everly in 1990, so when GNR opened for the Stones in 89, he was already engaged. I'm just assuming here, but I doubt he would've brought a date a few months before he got married.

He was never "engaged" to Erin Everly.  He came to her house at 4 in the morning on April 28, 1990 and said that if she didn't marry him he would commit suicide.  So they went to Vegas that same day and got married.

I don't know if he cheated on Erin or not, you maybe a very reliable source to find that out since you are his wife ? , but it wouldn't make any sense to make a public appearance with somebody from the opposite sex other than your fiancee/girlfriend. But then again, nothing has ever made sense in GNR.
