Please excuse my scan job and anyone willing to fix it up please let me know. I had to do it in two pieces and put it back together because it was too large for the scanner. I just thought that someone who went who doesn't live in the NY area might want this. Also I put it on megaupload because I left the image fairly large for anyone wanting to make adjustments. Enjoy.
