That pic is from an old rolling Stone interview that slash did not too long before Use Your IIIusion was to come out. It was a great read actually. Maybe the rolling Stone website could help out in some way.

VistaPrint. You can design a shirt on their website and order as few as you want (not just in bulk).

Before designing too much make sure that they will accept the picture. They know that you don't have a copyright for it and probably will reject it. It's happened to me but not in the US though.
